Estrogen Management: Controlling E2 on Testosterone Therapy
Aromatase inhibitors and SERMs to manage estrogen on TRT.
Drug Monographs
Anastrozole
Arimidex
An aromatase inhibitor (AI) used to control estrogen levels in men on TRT. When testosterone is administered, some converts to estrogen via the aromatase enzyme. Anastrozole blocks this conversion, preventing estrogen-related side effects like gynecomastia, water retention, and mood issues. Typically used at low doses 1-3 times per week.
Tamoxifen (Nolvadex)
Nolvadex
A selective estrogen receptor modulator (SERM) used in TRT protocols primarily to prevent or treat gynecomastia (breast tissue growth). Unlike anastrozole which blocks estrogen production, tamoxifen blocks estrogen at the breast tissue receptor while allowing estrogen to function normally elsewhere. Often preferred over aromatase inhibitors when estrogen levels are not excessively elevated but gynecomastia symptoms are present.
