Hair Loss Prevention: Managing DHT-Driven Hair Loss on TRT
5-alpha reductase inhibitors to prevent DHT-driven hair loss on TRT.
Drug Monographs
Dutasteride
Avodart
A dual 5-alpha reductase inhibitor that blocks both Type I and Type II enzymes, reducing DHT by approximately 90% - more than finasteride. Used off-label for hair loss prevention during TRT and on-label for benign prostatic hyperplasia. Its very long half-life means effects persist for weeks after discontinuation. Generally reserved for men who don't respond adequately to finasteride.
Finasteride
Propecia (1mg), Proscar (5mg)
A 5-alpha reductase inhibitor that blocks the conversion of testosterone to dihydrotestosterone (DHT). Commonly prescribed alongside TRT to prevent or slow hair loss, since exogenous testosterone can accelerate male pattern baldness. Reduces DHT by approximately 70%. May also reduce prostate size in men with BPH. Available in low-dose (1mg) for hair loss and higher dose (5mg) for prostate.
